Job Title: Case Manager

Reporting Relationship: Position reports directly to the Homeless Services Program Director

Purpose and Scope: To provide case management services and financial assistance to clients who are eligible through the various housing programs and grants.

Job Responsibilities and Essential Functions:
Case Manager

  • Plan, coordinate, and advocate for homeless or at-risk individuals.
  • Screen clients for eligibility according to specific grant financial requirements and vulnerability index.
  • Participate in the homeless coalition’s coordinated entry and assessment system process:
    • Serve as an access point for the homeless coalition’s coordinated entry and assessment system.
    • Administer vulnerability index screening tools.
    • Participate in coordinated entry and assessment client staffing meetings.
    • Accept clients through the homeless coalition’s coordinated entry and assessment system.
  • Coordinate the provision of services afforded by available grant funding sources, including but not limited to the HUD Rapid Rehousing grant, PATH, EFSP, State of Florida Challenge Grant, Emergency Solutions Grant, and Homeless Prevention Grant (TANF).
  • Participate, as necessary, in conducting home inspections required by the Emergency Solutions Grant and HUD Rapid Rehousing Grant.
  • Maintain client case files.
  • Complete monthly reporting requirements for grant sources, as appropriate.
  • Become SOAR certified.

2-1-1/Information and Referral (I&R) (3%)

Refer clients to appropriate resources using the 2-1-1 database and/or I&R databases for needs presented which are not related to housing

General Duties Supporting United Way (2%)

  • Responsible for participating in the Veteran By Name List committee.
  • Process MISSION UNITED clients.
  • Pursuant to HB 7165 regarding Early Learning, secure and submit Early Learning Coalition referrals for households identified as homeless.
  • Complete mandatory training, including HIPAA.
  • Participate in the System Performance Committee.
  • Send updated resources to coordinated entry.
  • Complete Rent Reasonableness forms.
  • Participate in Point in Time activities.
  • Perform additional projects and assist with special events as assigned.

Knowledge, Skills and Abilities:

  • Excellent organizational and planning skills; ability to prioritize and manage multiple assignments; strong attention to detail.
  • Excellent written and verbal communication.
  • Ability to develop strong trusting relationships in order to gain support and achieveresults.
  • Ability to legally operate a motor vehicle and provide own transportation.
  • Ability to maintain confidentiality.
  • Strong computer skills (Microsoft Excel, Word).

Education and Experience:

  • BS/BA in Social Work, or related preferred.
  • 1-2 years’ experience as a case manager.
  • Any equivalent combination of experience and education from which comparable knowledge, skills and abilities have been achieved may be acceptable.
